As far as I know, we were actually planning on getting rid of the formal [warn] thing in game.

I was on a faction server, and when someone broke a rule they would get warned with a command. After 5 warnings within an hour (or however long you set it) they would get a small tempban from the server. It also kept a record of all the warns, with the reason, date, and time
