Suggestion:
Create a ban request form rather than having it go under complaints or appeals. The ban requests would still be lumped in with the complaints, unless you really feel it's necessary to make a third section.
Reason:
It's just silly that people have to call their threads [User Complaiant] rather than [Ban Request] and have to file an improper format.

Do you wish to be temporarily banned for three days?: Yes, I do. (Normal text field, and the words below ask if the user wants to explain why)
Do you understand that this ban is un-appealable?: I understand.

Live with it? I don't see why this shouldn't be implemented, it's a simple fix and can only help staff figure out exactly what the complaint is for.
