This particular tool was created/owned on March 30th, and it is indeed a Melon Sword.
There haven't been any movement logs for this ID since April 24th, nor can I find any indication the tool still exists across any of the Main's.

This has happened to me too as well. It’s happened 5 times now. The first time I freaked out and panicked and started spam clicking the place where the sword was and it reappeared. Now when this happens I just click the place in my hot bar where the sword was and it shows back up.

This is a visual bug on your clients end.
You can however provide exact reproduction steps and perhaps we'll be able to push a fix for it.
Do so in a new help/support thread.
