Brief Description: Warped to tutorial area and the cosmo menu chest item overwrote the magic pick (#LK7482UQ) in the 1st slot of my hotbar
Instructions:
1. Have item in 1st slot of toolbar
2. do /tutorial
How many times did you recreate this?: Twice, @314 was able to recreate as well
Result: The cosmo menu chest item will overwrite the item in the 1st slot of your hotbar
Also note: relogging will leave the 1st slot empty upon logging back in
Expected Result: The cosmo menu chest item would not overwrite my magic pick. presumably it would disappear when leaving the area

Okay, so I did some testing. These steps in particular do not trigger the bug for me and this is also not what happened in your video. Neither /tutorial nor /warp tutorial add a cosmo chest to my inventory. However, the following two paths work 100% reliably for me:
Path 1 - /home (Tested as Pivillean)
- Ensure that you do not have any homes set.
- Use /home, which teleports you to the tutorial as a fallback location.
- This will override the first slot in your inventory with the cosmo chest.
- Ensure you have the Tombstone feature so that you keep your inventory upon death.
- Die. Thanks to tombstone, your inventory still has all of its items while you are on the death screen.
- Upon respawning, most of the inventory will be preserved, except the first slot will again be replaced with a cosmo chest.
- Ensure that you do not have any homes set.

/tutorial yields inconsistent results for me, cannot recreate now but had the first couple attempts out of 10 or so give me the cosmo chest using /tutorial.
Using /home when you have no homes set will result in this bug 100% of the time for me as 314's testing showed and this is what occurred in the video -

Should be fixed, I've removed cosmos for the time being as the plugin is fucked.
Please attempt your reproductions and let us know if it still happens for you.
@ClarinetPhoenix will replace any of your lost items after she does some investigating into what they were exactly and blacklists the old ones. Please message her with ownership ID's of the items that were lost.-

Can confirm. Dying and using /home without set homes no longer summon a chest and everything seems fixed now.
