I am getting this as well and am also getting the generic avatar icon when previewing the profile (larger image.)

but screws up in the lightbox and profile page:

 

I'm getting this as well, and just a moment ago I was viewing my profile and I had the genderless default picture (Skeleton with ECC text), I refreshed and it changed to the generic avatar.

 

avatar_s works fine, but avatar_m and avatar_l don't.

 

Oops, update issue I believe with my recent theme upgrade.
Should be fixed now (or within 15 minutes) - clear your cache, etc.

Yeah, forgot to "not-override" default avatar images.
No clue why avatar_s still worked with our custom one though, lol wtf?

 

I had to clear my cache, but it is fixed.
