Yes the warp can be anywhere within the same town as the portal. Only 1 warp per town.
Warps must be put in the same town the portal was in.
The portal stops working and you keep the warp. You may contact a GA+ to remove the portal sign if you wish.
You don't need to file the application. If you would like the portal sign removed contact a GA+ via forum PM with coords and world.
