The correct format for this is the following: Create a thread titled "Ban Appeal", and copy/paste this into it. You can then fill in the questions. Remember to be as descriptive and detailed as possible when creating this. DeVaHz may appeal his ban once you have been unbanned. Not a complaint -- Locked.
