Do you have /fly on or off when this happens? If you have it off, lava doesn't absorb fall damage, so you may take fall damage when you it the bottom of the lava.
I have it on 95% of the time, and the times I don't and jump in lava, it doesn't hurt as my hot tub is 1 deep with 1 blocks to jump from. Also I was sitting still the times it did happen. I occasionally test it. It seems fine lately
