Yesterday, I was preparing to have my Efficiency III, Silk Touch I diamond pickaxe repaired by a trusted person (jassimq). However, when I threw the pickaxe, he never picked it up. You might have been in the way of a unlucky butcher. Perhaps when you threw it on the ground, a auto-butcher happened killing that object.
Nothing can be done, the item is worth a few thousand at most. I suggest next time you maybe put it in a chest for the player to take out.
