Dude. I need you to take a video. This kind of report is saying "Bug happened. I don't know how to do it and I can't show it"
There is no way to recreate this bug no matter how hard I try to. It has happened to me once in the end and once in the nether. I think it may be a server side bug. Next time it happens i will be sure to document it.
