This is called butcher. Every 10-15 mins (not exactly sure the amount of time) entities are cleared in the mining world to help reduce server lag. Sorry about losing your tool. Edit: I recommend going to a main world for e-repairs, or placing down a chest while in the mining world and transferring the item through that.
I have had this happen twice when E-Repairing people's tool. I don't remember the other person's name, but they tossed it to me, I repaired, then tossed back and it was gone before they can pick it back up. This occured both times in the town worlds so it couldn't have been butcher. The last time this happened we provided Dewsy the id and I was told he said he couldn't find it. Also, @ASilverFox died in a pit in his house and his tools and stuff was gone when he went back to get them. This may be do to the same glitch if it is a glitch.
(P.S. I would reccomend sending the id to Dewsy or posting the item id on here for him to make it easier to look into.)
Since it was in the main worlds... I remember a bug with instantly disappearing items from singleplayer. Might be the same - but if it is, it should be unfixable for ECC. ~ALWAYS use chests...

I lost my beloved Eff7 "Squeege" the same way. I put it in a chest (main world) or so I thought but instead it landed on the floor and I didn't realize it. Came back a little while later and it was gone. Dewsey investigated and even narrowed it down to the block where it disappeared. Now that was a heartbreaker!
There was no way to use a chest? Other tips?... Wait until you are on land since you shouldn't be using the tool until you hit land anyway. Leave the town since you aren't a member of it and can't use the tool without being a member. Any other reason a chest couldn't be used? Oh and ALWAYS use chests when dealing with other players and repairs.
I think his case was a death and subsequent loss of items to butcher rather than chests. Only way to prevent that is to buy the ever-buggy tombstone...which is pricy.
