Death in water in other worlds doesn't spawn tombstone as well as in lava. IMHO that's because the algorithm can't find empty blocks in which to spawn the chest. I think they are different bugs, one specific to nether and one not
According to the ECC Shop page. Tombstone is never intended to spawn in the nether. It does keep my exp, which is as intended Not a bug.
Being TombstonePlus an upgrade of Tombstone, what I understand is that it extends the feature to nether. If it is never intended to spawn the chest in nether, but only to save xp, then this should be written explicitly in the description. Being as it is, it's misleading.
