I don't understand, where do I put my ban appeal and what is the server wiki I thought I did this right I stated the rule violated and I said what it meant please explain what I did wrong, I'm very confused.

Until you do this I cannot go any further with your appeal. If you do not understand still try looking at other ban appeals and see what they did so that it helps.
