To Clarify: As your Forum name is Different than your in game name that you were last seen on the Server, you will need to Submit a Forum Name Change Application found here. Once your Forum name is changed and matches your last seen in game name, we will then be able to Proceed with the Appeal In this case you will be Requesting to change your Forum Name to CaptainBroski.
